Instructions: As part of the independent evaluation of the Title IID Technology Initiative, we are interested in learning about how you are working with teachers and other school personnel to support the use of NYLearns and the implementation of technology. Each time you provide support or training on the use of technology, describe the event by completing a line of the table below. Where appropriate, use the code(s) provided on the bottom half of the form. The first line has been completed as an example. Thank you.

BPS technology Support/Training Log
INSTRUCTIONS
Purpose of the Support/Training Log: The Support/Training Log was created by MAGI Educational Services, Inc., the independent evaluator for the Buffalo/Niagara Title IID Initiative. It was designed to capture a snapshot of the type and content of technology training taking place in participating schools, and is an important component of the overall evaluation effort. Please be assured that all data will be treated confidentially, and all respondents will remain anonymous when data is reported.
How and When to Complete the Log: The Support/Training Log needs to be completed by all Technology Instructional Staff (TISs). Please complete one line of the log form each time you conduct a training/support event or session on the use of technology, either formal or informal (e.g., small group/individual meetings, classroom demonstrations, workshops, etc.) Record all events from April 18 through May 13.
Column A: Date of Event—Indicate the day that the event took place. You may have more than one entry for a particular day. For each event date entered, complete rows B through I as they pertain to the event.
Column B: Schools Represented—For each event entered in Column A, indicate which school(s) you worked with by entering the school number. Multiple schools may be listed where appropriate.
Column C: Audience—Indicate the role(s) of the staff members with whom you worked.
Column D: Activity—Indicate the activity, or delivery method, of the training/support event.
Column E: Content—Indicate the focus of your work with staff by identifying the content area of the support/training.
Column F: Used NYLearns—Check “Yes” or “No” to indicate whether the NYLearns web site was used or addressed as part of the training.
Column G: Subject Area—Indicate all curriculum areas that were covered in the training.
Column H: Duration—Indicate how long the training event lasted, using 15 minute intervals (enter .25 for 15 minutes, etc.)
Column I: Number of Attendees—Enter the number of participants, excluding yourself. There may be as few as 1, for a one-on-one session.
